Licensed Clinical Social Worker (School based program affiliated with an FQHC) Chicago, IL Loan Repayment through the NHSC! We are seeking a Licensed Clinical Social Worker to work as a school based LCSW for a Community Based Mental Health Center in Chicago, IL. The Provider would receive a competitive salary of $60,-$65,+ (Depending Upon Experience) with a comprehensive benefits package including Medical, Dental, Vision, K, Life Insurance, PTO, and CEU allowance. This location is a loan repayment eligible site through the NHSC and would provide $25, per year to the LCSW candidates student loans (minimum of a 2 year commitment required in exchange for $50, in loan repayment). $1, signing bonus and relocation assistance available. This position provides Behavioral Health consultation to children, adolescents, adults and families in order to improve psychosocial functioning. Responsibilities

Provide comprehensive assessment and diagnosis of Behavioral and Mental Health clients. Develop effective treatment plans, that include specific goals to be met and timeline for meeting goals Assist clients in achieving established self-management goals Actively facilitate communication with parents/guardians and families Actively facilitate communication with teachers and other school personnel, including school social workers, counselors, behavior team, special education, and administrators Evaluate crisis situations and apply appropriate interventions or referrals if needed Actively participate in meetings that support an integrated Medical/Mental Health care model to provide comprehensive care for clients Assist in the detection of at risk patients and development of plans to prevent further psychological or physical deterioration Assist in the management of chronic disease by working with patients in self-management of chronic diseases Refer patients for other treatment when objective progress in patients condition cannot be documented Participate in quality improvement documentation, including the use of symptom rating scales and peer review evaluations, as necessary Provide trainings for staff and/or families on Behavioral Health issues as needed Other duties as assigned Qualifications

Computer skills - knowledge of word processing software Excellent working knowledge of Behavioral medicine and evidence-based treatments for Mental Health and Behavioral Health conditions Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of Mental Health and Behavioral Health conditions Ability to communicate and work with an interdisciplinary team in a primary care center serving large numbers of low income and minority patients General knowledge of the clinical effects and side effects of prescription and illicit drugs used by Mental Health and Behavioral Health patients Ability to design and implement clinical pathways.
