We are looking for an
Addiction Specialist
to join our
Program for Assertive Community Treatment (PACT) , a highly integrated, forensic-focused outreach and 24/7 residential program that reduces hospitalizations and incarcerations while supporting individuals in recovery and wellness.
What You’ll Do As an Addiction Specialist, you will play a key role in supporting individuals on their recovery journey, providing clinical, therapeutic, and case management services to help them achieve stability and independence.
Key Responsibilities
Delivering community-based services within the PACT forensic model, including therapy, medication administration, ADLs support, and care coordination
Conducting substance use evaluations, creating recovery plans, and providing crisis/risk assessments
Facilitating dual-diagnosis recovery groups and linking individuals to AA, NA, detox, or medication-assisted treatment programs
Offering outreach services in homes, hospitals, and community settings
Serving as a primary case manager for an assigned caseload, ensuring individualized, coordinated care
Consulting with families and community agencies to enhance support systems
Maintaining program documentation, treatment plans, and progress notes in compliance with agency standards
Providing safe and timely transportation for appointments, activities, and daily living needs

Who You Are
Bachelor’s Degree in a related field and/or CADC certification (required)
At least 6 months of experience in human services (preferred)
MAP certification or willingness to obtain within the introductory period (required)
Willingness to work with diverse and forensic-involved populations within the Sequential Intercept model
Strong cultural competence and commitment to empowerment, human rights, and community integration
Working knowledge of community resources and how to connect individuals to supports
Valid driver’s license and reliable vehicle available during working hours (required)
