Personal Care Attendant (PCA)
works as part of an
interdisciplinary team
to implement individualized care plans, ensuring high-quality personal and supportive care both in the participant's home and at the Adult Day Health Center. This is a capitated health plan, offering a stable and thriving environment.

8:00 am - 5:00 pm Compensation:
$23 - $30/hr , depending on experience
Core Responsibilities The PCA provides essential
Activities of Daily Living (ADL)
and
Independent Activities of Daily Living (IADL)
support, following the participants care plan and PACE regulations. Direct Participant Care:
Provide hands-on assistance with personal care, including toileting, bathing, dressing, grooming, feeding/assisted dining, meal service, laundry, transfers, ambulation, and range of motion exercises. Safety and Mobility:
Complete safe
transfers
utilizing Durable Medical Equipment (DME) as required (e.g., lifts, gait belts, Hoyer lifts). Documentation:
Accurately and promptly document participant services provided in the
electronic health record (EHR) . Hygiene and Environment:
Maintain a clean and orderly environment, including the participant's home or day center areas. This involves cleaning bathrooms, sanitizing showers, and relevant DME. Reporting:
Promptly report any
changes in a participants condition
to the supervisor or designee. Advocacy & Compliance:
Act as a participant advocate, reporting any policy violations, and maintaining the
confidentiality
of all participant information according to
HIPAA
requirements. Service Coordination:
Provide appointment reminder communications and, as needed, escort participants to company vans or outside medical appointments. Safety:
Maintain a safe environment by displaying awareness of hazards, and promptly reporting/reversing them in accordance with OSHA safety guidelines.
Day Center Specific Duties Assist with
individual and group activities . Support the
food service program , including serving snacks and meals, checking food temperatures, reviewing diet orders, and performing kitchen duties (washing dishes, sanitizing work areas, etc.). Must be able to obtain and maintain a
Food Handler's certification . Ensure that all wander-risk participants are wearing required tracking devices. Must-Have Skills/Qualifications Minimum
one (1) year of experience
working with frail, low-income elderly is strongly preferred. Demonstrate a
professional demeanor
and possess exceptional
patience
when working with patients. Able to
communicate clearly
and easily, ensuring full understanding by patients. Possess
good written skills
for accurate and timely charting of daily activities in the EHR. Required Competencies Ability to display a courteous, helpful, and professional attitude. Willingness and ability to be responsive to all customer groups, including team members, participants, and their families. Knowledge and skills necessary to provide personal care and ADL/IADL care to the frail elderly.
