Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the Elderly) organization that uses an interdisciplinary team approach to care planning and care implementation to provide high-quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promotes autonomy and quality of life.
Position The Personal Care Attendant I provides assistance to CEI participants in their home or day center. The PCA demonstrates knowledge and skills necessary to provide personal care and ADL/IADL care to frail elderly who need help continuing to live safely in the community. Services are provided as determined by the participant’s care plan in accordance with PACE regulations.
General Duties and Responsibilities
Provides Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL) such as toileting, showers, bathing, dressing, grooming, feeding/assisted dining, meal service, laundry, transfers and ambulation, and range of motion.
Completes safe transfers utilizing durable medical equipment (DME) such as lifts, gait belt, and hoists.
Cleanse and empty the participants’ bathroom, including commode.
Provides appointment reminder communications to participants.
Maintains a clean and orderly center or home, including kitchen, activity areas, and sanitizing showers/bathrooms and relevant DME.
Escorts participants to CEI vans when needed.
Documents participant services in the electronic health record accurately and on time.
Acts as a participant advocate by reporting violations of the Participant Bill of Rights.
Responds to participant complaints in a timely manner and documents complaints or requests appropriately.
Reports changes in a participant’s condition to supervisor or designee promptly.
Demonstrates dependability by arriving consistently and on time for scheduled work hours and submitting accurate timesheets.
Maintains a courteous, helpful, and professional attitude on the job.
Complies with all agency training requirements.
Maintains professional affiliations and any required certifications.
Provides a safe environment for participants and self by displaying awareness of safety hazards and reporting or reversing hazards.
Participates in quality improvement programs as required.
Follows all OSHA safety guidelines and CEI policies and procedures.
Maintains the confidentiality of all participant information in accordance with HIPAA.
Performs other duties as required in a positive and helpful manner.
Department Specific Duties and Responsibilities – Day Center
Records services in required logs or electronic health record.
Assists with individual and group activities as necessary.
Accompanies participants to outside medical appointments.
Ensures that wander‑risk participants are wearing the code alert device.
Obtains and maintains food handlers certification.
Assists with the food service program, including serving snacks and meals at the day center, checking food temperatures, and reviewing diet orders.
Performs kitchen duties such as washing dishes, sanitizing work areas, maintaining supply inventory, and checking for food expiration dates.
Department Specific Duties and Responsibilities – Home Care
Completes vital sign monitoring.
Provides medication prompts, including supervision and double-check of self-administration of insulin.
Emptying and cleaning of foley catheters, ostomies, and urostomies.
Performs dressing and grooming, including shaving, oral and skin care, monitoring skin condition, positioning, and turning.
Assists participants in completing home exercise programs.
Plans and prepares light meals and cleans up, storing and monitoring food expiration dates.
Performs light housekeeping tasks such as dusting, vacuuming, garbage removal, and recycling.
Performs laundry services including washing, folding, bed-making, and changing linens.
Follows the home care service plan to complete tasks.
Qualifications
High School Diploma or equivalent (preferred).
A valid California driver’s license with current vehicle insurance and reliable vehicle is required for Home Care work.
Current Basic Life Support CPR card and First Aid card required.
Proficiency in spoken and written English required.
Basic computer skills required.
Minimum one year of experience working with frail, low-income elderly preferred.
Ability to work independently with minimal supervision and as part of a multidisciplinary team required.
Bilingual preferred but not required.
Physical Demands The employee is regularly required to stand, talk, and hear, demonstrate manual and finger dexterity, and eye‑hand coordination. The position requires frequent stooping, kneeling, crouching, reaching, walking, and sitting. The employee must regularly lift, carry, and move up to 50 pounds and push or pull objects exceeding 150 pounds using appropriate body mechanics. Specific vision abilities required include close vision, distance vision, peripheral vision, color vision, depth perception, and ability to adjust focus.
Work Environment The employee encounters a diverse range of health conditions, environmental elements, and other factors inherent to providing care for vulnerable seniors in healthcare settings and residences.
