Job Summary And Qualifications We are currently looking for an ambitious Clinical Rehabilitation Specialist to join our team on a full-time/part-time/PRN/per diem/as-needed basis. A Clinical Rehab Specialist acts as a coordinator between internal/external referral sources and acute inpatient rehab operations.
Responsibilities
Establishing relationships with case managers, physicians, payers and other referral sources.
Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both the hospital case management team and the Rehab Medical Director.
Work closely with the hospital case management team to ensure a responsive and smooth-flowing referral and admission process for the inpatient acute rehab program.
Requirements
Graduate of an accredited school in clinical specialty.
Minimum 2 years recent clinical care experience.
Licensed Professional in a clinical health care specialty (i.e., RN, Physical Therapist, Occupational Therapist, or Speech Therapist).
Pass Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification.
If RN, CRRN preferred and/or achieved within 2 years of hire.
BCLS certification (AHA).
Clinical experience in the care and management of Inpatient Rehabilitation patients.
Experience as a clinical liaison coordinating admissions for Inpatient Rehabilitation patients.
