The speech pathologist/audiologist (SLP) is a qualified therapist who directs, supervises, evaluates and provides speech therapy services to patients in a home health setting. Responsibilities
Care Plan Development: Create, review and update individualized patient care plans addressing specific speech and language needs. Therapy Services: Deliver speech therapy services and activities for patients with speech and language disorders. Documentation and Patient Records: Create and maintain patient records and clinical notes in a timely manner. Collaboration and Development: Attend and participate in case conferences with local teams monthly, and actively participate in staff development opportunities. Communication: Communicate between the agency, clients and Area Support Manager to ensure a well-coordinated approach to patient care. Benefits
