Job Summary The speech pathologist/audiologist (SLP) is a qualified therapist who directs, supervises, evaluates and provides speech therapy services to patients in a home health setting.
Responsibilities
Care Plan Development: Create, review and update individualized patient care plans addressing specific speech and language needs.
Therapy Services: Deliver Speech Therapy services and activities for patients with speech and language disorders.
Documentation and Patient Records: Create and maintain patient records and clinical notes in a timely manner.
Collaboration and Development: Attend and participate in case conferences with local teams monthly, and actively participate in staff development opportunities.
Communication: Communicate between agency, clients and your Area Support Manager to ensure a well‑coordinated approach to patient care.
Requirements Qualifications:
Master’s degree in Speech-Language Pathology from an accredited program.
State licensure as a Speech‑Language Pathologist in Texas, or actively in process toward obtaining one.
Certificate of Clinical Competence in Speech‑Language Pathology (CCC‑SLP) from the American Speech‑Language‑Hearing Association (ASHA).
