We are looking for a passionate and dedicated
Case Manager
to be a key part of our team, committed to supporting clients’ comprehensive health and wellness needs. In this role, you will provide culturally and linguistically tailored services to help individuals navigate complex health systems with ease. Your focus will be on ensuring clients can access a wide range of vital services, including pharmacy support, cancer related services, physical and dental care, medical treatment, and community health services, while seamlessly integrating these with behavioral health care.

RESPONSIBILITIES
Assist clients in navigating healthcare systems and accessing a variety of services, including pharmacy support, physical health, dental care, and community health resources. Provide support across all social determinants of health, including transportation, language interpretation, assistance with filling out forms, and other essential services.
Facilitate connections between these services and behavioral health care, ensuring a cohesive, integrated care approach.
Accompany clients to appointments when necessary, providing in-person support.
Act as a cultural mediator, bridging Western healthcare terminology and processes with the cultural norms of both Members and Nonmembers.
Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care delivery.
Advocate for clients' needs, providing guidance and empowerment to improve health outcomes.
Educate clients on how to schedule their own medical appointments, manage transportation independently, and overcome barriers to accessing healthcare and other services.
REQUIREMENTS
3 years of related experience; case management in mental health or other related health care field.
HS diploma or equivalent
QMHA is required within 30 days of hire.
Fluency in English
Intermediate to advanced mastery of business applications and productivity suites
Demonstrates resilience, awareness and self-regulation in navigating challenges, conflict and obstacles/barriers.
Thrives in a diverse and multicultural environment; works with cross-functional teams to develop and implement strategies to enhance end-user experiences throughout the service lifecycle
Demonstrates exceptional commitment to serving and advocating for diverse and marginalized populations
Successfully completes tasks in a low noise environment
Interact with others and perform tasks in-person and through the use of technology, with moderate periods of stationary activity and moderate periods of increased physical activity including and not limited to standing, walking, sitting.
Moves equipment/materials weighing up to 25 pounds
Travels between worksites regularly; responsible for transporting clients in personal and/or agency vehicles, which requires valid driver’s license, auto insurance, and motor vehicle records clearance
May arrange for transportation for clients in addition to interpretation services.
Works in a variety of settings, including and not limited to LCSNW office locations, partner agency and community-based settings, client home/community settings, outdoor settings in varying weather conditions
Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program

Vacation includes up to 2 weeks in the first year.
