Base pay range: $20.00/hr – $22.00/hr
Position: Part-time, 2nd shift, 16 hours, Saturday & Sunday (2:30–10:30 p.m.)
Program Summary
During the 3–6 month length of stay in this congregate care setting, pre‑trial adult males with a primary mental health diagnosis are provided with case management, supervision, and psycho‑educational and skill‑building services to support them in re‑entering the community after incarceration. The goal is to reduce recidivism through positive behavioral change and pro‑social community connectivity.
Position Summary
The Case Manager 2 provides case management services, including intake assessment, risk/needs assessment, psycho‑social education, development and implementation of individual service plans, discharge planning, and liaison with treatment providers and community resources. The Case Manager 2 monitors client compliance with placement conditions and stipulations, carries a caseload, implements and facilitates resident activities/interventions, and provides program shift coverage.
Working within Adult Services, the Case Manager 2 works on the Residential Behavioral Health Treatment care team to support clients’ recovery from mental illness, substance use, and homelessness.
Requirements
High School diploma or GED
3 years related experience in the direct service and case management field
Experience working with SPMI population in a residential setting and with individuals re-entering the community after incarceration
Valid Connecticut Driver’s License
Regular and predictable attendance required
Orientation If selected for employment with The Connection, newly hired employees will be required to attend a two‑day mandatory New Hire Orientation at our Roscommon office located in Middletown.
