The incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. The nurse assists with procedures, patient education, outpatient and post-hospital discharge follow-up, including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse integrates psycho-motor and age-appropriate cognitive skills of the patient/family/significant other using knowledge of assessment, planning, intervention and evaluation. Responsibilities
Provide direct patient care and triage (phone and in-person) in a clinical setting. Assist with procedures and patient education. Coordinate outpatient and post-discharge follow-up, including home care, equipment, and therapy services (PT/OT). Apply psycho-motor and cognitive skills appropriate to patient and family needs; perform assessment, planning, intervention and evaluation. Qualifications
Required Qualifications
Minimum of 6 months nursing experience in an acute care or ambulatory care setting. Current California RN license. Bachelor of Science in Nursing (BSN) required for external candidates. Ability to work variable shifts including evenings, nights, weekends, and holidays. Effective English communication skills (reading, writing, speaking). Flexibility to adapt to a changing environment and workload; ability to establish and maintain effective working relationships within the Health System. Preferred Qualifications
Understanding of applicable regulatory requirements. Cardiovascular clinical experience. Bilingual skills to communicate with patients and families. Licenses and Certifications
Basic Life Support (BLS). Education
