We work hard to keep the territory manageable as well as see the patients timely. As a Case Manager, you can make an impact in our patients lives and create an inclusive collaborative team model by organizing, communicating , and aligning a multidisciplinary team.
Our Case Manager RN provides highly skilled nursing care in the home setting as the admitting nurse, providing services such as wound care, IV medication administration in addition to supervising LVN care delivery and care coordination with the multidisciplinary team. The Case Manager is the front line clinician communicating with the physician overseeing the plan of care. As a Case Manager your role is integral to leading the case conference and improving clinical outcomes.

Qualifications
Minimum: One year experience as a RN Graduate of an accredited registered nursing program California Registered Nurse license. Current AHA BLS certification California driver’s license with proof of liability insurance for automobile.
