Overview
Clinical Navigator - Transplant-20617 at Rush University Medical Center. Location: Chicago, Illinois. Work Type: Full Time (Total FTE between 0.9 and 1.0). Schedule: 8 Hr (7:00 AM - 4:00 PM). Summary: Responsible for assisting providers and registered nurses with the evaluation, management, and treatment of patients in an ambulatory care setting. Maintains patient flow and exam/treatment room upkeep. Performs basic skills such as vital signs and point of care testing. Administers injections, immunizations, small volume nebulizer treatment, and over-the-counter medications under physician direction. Facilitates outreach site needs and serves as a liaison from outreach clinics. Represents Rush mission, vision and values and adheres to Rush policies and procedures and Customer Service Standards. Responsibilities
Coordinates and maintains organized patient flow. Performs patient intake: vital signs, height and weight, reason for visit, interim history, allergies, medication lists, and other required interventions. Prepares patient for exam based on reason for visit. Prepares and supports patient and assists provider with procedures. Reviews appointment schedules and ensures all patient charts are available for the visit. Reviews patient charts and ensures ancillary reports and required paperwork are available to the provider at the time of visit. Performs basic lab and testing procedures according to standards. Performs phlebotomy and EKGs. Administers immunizations as ordered by Physician. Administers Rush University Medical Group approved intramuscular or subcutaneous medications after competency assessment. Performs nebulizer treatments in the clinic setting after competency assessment. Performs Tuberculin skin testing as ordered by the Physician. Documents administration of medications in Epic per policy. Calls patients regarding test results and medical instructions under physician direction. Calls in medication refills under physician or protocol direction. Documents patient care information in the outpatient medical record per standards. Assists with scheduling of patient tests, procedures, and follow-up appointments. Maintains cleanliness and sterilization of equipment and instruments; inventories medical and linen supplies; maintains quality control for equipment. Provides cross coverage to other clinical areas as assigned and participates in quality improvement projects. Maintains compliance with The Joint Commission (TJC) standards and other regulatory bodies. May have unit-specific duties as needed. Ensures timely follow-up to referring providers at critical points (letters, phone calls, emails, faxes). Ensures timely feedback to referral sources at critical in-patient points (referral, listing decision, transplant, discharge). Travels to offsite clinic locations to provide medical assistant care and represent Rush Transplant as a liaison for care. Conducts outreach and assessments to identify individuals at risk for adverse health outcomes, including Health Risk Assessments. Executes on-site visits to referral sources and physician offices to support growth plans for liver transplant. Facilitates outreach site needs, serves as liaison for feedback to Rush Transplant administration (equipment, technology, resource gaps/issues). Shares knowledge to empower patients to navigate services and systems; liaises with multiple departments, agencies and provider offices. Qualifications
3 years clinical experience required. 1 year of care navigation experience required. High school diploma/GED required. Graduate of an Accredited Medical Assistant Program required (unless hired prior to February 1, 2022). Current CPR certification required on start date. Clinical Medical Assistant Certification by AAMA, AMT, NCCT, or NHA required within six months of hire (includes injections and medication administration). Computer skills. Excellent communication, interpersonal, customer service and organizational skills. Preferred Qualifications
