Job Summary and Responsibilities
JOB SUMMARY / PURPOSE
The Clinical Pharmacist Ambulatory Services provides direct patient care in hospital or community-based clinics. The pharmacist works collaboratively with physicians, nurses, other healthcare providers, and patients to optimize drug therapy and outcomes using evidence-based standards of care. This job is responsible for providing effective, efficient and accurate ambulatory pharmaceutical care to achieve organizational, patient care, and quality measure goals and objectives. The incumbent is responsible for working within the scope of their license, performed in accordance with organizational protocols, internal guidelines/standards, and any applicable State regulatory requirements.
ESSENTIAL KEY JOB RESPONSIBILITIES
Participates in the evaluation and management of diseases and health conditions in collaboration with other health care providers
Prescribe pharmaceutical agents as outlined in the approved collaborative drug therapy management agreement including initiating, adjusting, and discontinuing drug therapy
Order and interpret drug therapy-related tests
Ensure patients have access to and receive safe, efficacious and cost-effective medication therapy
Work collaboratively to resolve medication access and adherence issues
Address medication-related issues to support care coordination and transitions of care
Performs appropriate Point-of-care laboratory testing as required for the visit
Provides patients and/or families with focused teaching on safe and effective use of prescribed medications and non-pharmacologic management of chronic health conditions
Precepts Pharmacy Interns and Pharmacy Residents. May serve as the Residency Program Director or Coordinator if applicable to their location.
Job Requirements
Graduate from an accredited school of pharmacy
Active KY Pharmacy license in good standing
Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies-Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served and infection control, safety and risk management standards as relates to the ambulatory pharmacy setting.
Preferred
Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities
Completion of a PGY2 Ambulatory Care Pharmacy residency
Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities
