Position: Case Manager 2 Program: Coley House Location: Hamden, CT Salary: $19.50/hour Schedule: Full time, 40 hours, 1st shift, Sunday-Thursday 7am-3pm A residential step-down program where pregnant and/or parenting women participate in substance use treatment while living in a safe and structured environment. Case Managers meet with clients weekly and facilitate daily psychoeducational groups to support clients in returning to their communities as healthy, productive parents in recovery. Position Summary

The Case Manager 2 provides staff support and direct client care for the women’s residential recovery short-term treatment facility and the men’s residential permanent supportive housing program in Bridgeport. Responsibilities include delivering comprehensive case management services, conducting intake assessments, developing and implementing individual care plans, and liaising with treatment providers and community resources to support client success. This role is non-exempt and works under the supervision of program management. Within Family & Child Services, the Case Manager 2 collaborates with Coley House staff to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness. Responsibilities

Provide case management services to clients, including intake assessments, development and implementation of individual care plans Liaise with treatment providers and community resources to support client success Deliver direct client care within the women’s residential recovery short-term facility and men’s residential permanent supportive housing program Support family stabilization and reunification efforts for clients facing complex issues Qualifications

Associate’s Degree 2+ years of relevant experience in direct service and case management Valid Connecticut Driver’s License Regular and predictable attendance Orientation
