Job Description Summary
Sojourn's mission is to provide, without prejudice, shelter and support services to adult and child victims of domestic violence; prevent violence through non-dominant child-rearing, and the re-envisioning of gender stereotypes using a philosophy of nonviolence and individual empowerment; and work toward advancing social justice, locally and globally. Through shelter, support groups, workshops, hotline calls, children's empowerment programming, legal services, and service referrals, Sojourn brings safety, connection, and hope to individuals and families. Sojourn also works to educate the broader community and advance public policy that affirms the rights of oppressed populations.
Job Description – Essential Duties and Responsibilities
Under the supervision of the Senior Program Manager, the Case Manager will: Maintain accurate progress notes and documentation of client services. Adhere to program/contract compliance as per guidelines and requirements mandated for DV shelter programs, standards of confidentiality, HIPAA, safety and crisis intervention. Assist with tracking and data entry as needed, including daily shift logs, census reports and other documentation. Train, assist and provide feedback to volunteers and Counselor Advocates. Provide direct services to clients: Provide case management (housing, medical & social service advocacy, job development, transitional housing, case planning, etc.). Provide housing navigation services to clients (advocacy with housing providers, assist with housing search, navigate HUD/EHV systems, etc.). Coordinate possible co-case management with other The People Concern projects/outside agencies. Conduct intake/assessments/screenings of clients. Assist with client crises as needed (life skills, conflict resolution, safety planning, etc.) using trauma-informed de-escalation and mediation techniques. Ensure implementation of empowerment model, harm reduction, and trauma-informed care methodologies. Provide crisis intervention, safety planning, resources and eligibility screenings for program services via Sojourn 24-hour hotline for DV crisis per best practices. Assist with client entry and exit of shelter programs. Assist with transportation of program participants to meetings such as court, doctors, community resources, and housing appointments as necessary. Facilitate support groups for Adult and Child Participants including life skills, DV education, and Parenting Education. Provide child-care occasionally. Ensure adherence to shelter guidelines for program participants under the supervision of shelter management. Provide resources and referrals as needed. Assist with life skills support and education (grocery shopping, chores/cleaning, child care); establish and maintain positive working relationships with clients, staff, community partners and stakeholders including mental health programs, DV shelter programs, local law enforcement, local neighborhood councils, and providers of services and resources to homeless persons. Attend staff meetings, case conferences, training workshops, and community meetings as needed. Additional duties include inventorying site needs, submitting facility maintenance, IT, and supply requests; submitting daily activity reports; communicating during shift transitions; participating in safety drills and facility checks; maintaining shelter cleanliness; reporting credible suspicions of child abuse; and other duties as assigned.

High School Diploma or GED or Equivalent Trustworthy, honest, and a team player Ability to supervise shelter participants with dignity and respect; engage participants positively and troubleshoot problems Sound decision-making ability related to crisis and safety planning Ability to work with a diverse population and implement cultural responsiveness Effective communication with the public, TPC staff and volunteers via phone or in person literate with basic computer skills; able to maintain confidentiality Able to supervise sanitary conditions in all areas Willingness to work flexible hours and handle emergencies as needed Understanding of Domestic Violence Survivor population and their needs Use of own vehicle; maintain required auto insurance in California Valid CA Driver's License, reliable automobile, and acceptable driving record Extensive experience in social services with direct case management experience preferred Willingness to receive 40-hour Domestic Violence Training
Job Description – Preferred Qualifications
Bachelor's degree in related field preferred but not required Minimum of two years in social services or case management; 24-hour shelter experience preferred Experience in communal living or shelter environment preferred Combination of field and office work Regularly required to sit, stand, bend; occasionally lift up to 35 lbs Occasional walking or driving to local sites Ability to work in busy and loud environments; exposure to elements May need to bend, stoop, twist, and sit throughout the day
