POSITION SUMMARY
The Multi-Dimensional Family Therapy Mental Health Counselor provides case management services in addition to recovery management checkups. The MDFT Mental Health Worker provides model adherent intensive in-home clinical care coordination to adolescents and young adults with substance abuse and delinquent behaviors and their families. This position maintains a positive and optimistic attitude and works collaboratively with internal and external providers to procure family needs and services. The Mental Health Counselor completes documentation in an accurate, timely, and compliant manner consistent with regulatory guidelines to contribute to the financial viability of the program.

Salary $48,000
EDUCATION AND EXPERIENCE/QUALIFICATIONS
BA/BS degree is required plus a minimum of 3 years experience in the human services field is preferred. Must have a valid driver’s license and automobile. Requires basic computer skills. Demonstrated orientation toward serving people with substance use and/or mental health issues in their community preferred.
Current program needs require the ability to fluently communicate in English and Spanish.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides coordination for referrals as well as establishes and maintains referral agreements and linkages with community referral sources or ancillary service providers.
Provides group and/or individual counseling to clients with appropriate supervision and coordinates treatment phases both internally and with contracted or external service providers.
Provides direct case management services and develops case management plans specific to each assigned client and family, including interventions necessary to increase the probability of treatment completion, reducing/maintaining sobriety, and to improve life functioning.
Maintains records of case management activities for reporting and tracking purposes and completes all documentation for client medical records in accordance with documentation timelines and in compliance with Medicaid, licensing and other accreditations (e.g., treatment plans, treatment plan reviews, and progress notes).
Attends team meetings, staff meetings, or rounds to assist in coordinating case management and review of treatment plans.
Audio recording of support sessions as required and uploading into online portal.
Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate. May provide translation to support clinical care as appropriate.
Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.
Administers standardized screening tools and collects data as defined by program, as well as collecting urine screens as required.
Provides/supports safety planning, relapse prevention plans and crisis intervention with appropriate supervision and as defined by program. This may include after hours on-call responsibilities.
Provides recovery management checkups and support to clients and their families in person, over the phone, and in the community (e.g., provide support and ongoing assessment, facilitate involvement with pro-recovery peers and activities, detect relapse and other concerns, assertively link to services as needed, and promote positive family relationships) post discharge.
Attend and participate in monthly coaching calls with Chestnut Health Systems. Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
