The Mental Health Counselor provides direct services, case management, and coordination of care to an assigned caseload of individuals and families. This position assists in accessing needed supports and resources in the community and facilitates ongoing communication and advocacy between the family, DCF, school, probation, and other involved supports or service providers. The role may also facilitate or co-facilitate groups in accordance with standard practice. Essential Duties And Responsibilities
Develops case management plans or plans of care and provides direct case management services and/or coordination of care specific to each assigned client and/or family, including interventions to increase treatment completion, build on client/family strengths, and improve life functioning. Submits and reviews documentation as defined by the program. Demonstrates knowledge of psychopathology and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements program-specific safety and consultation protocols. Uses best practices and evidence-based practice models where appropriate and adheres to model and program expectations where applicable. Provides front-line crisis support to individuals and/or families in the program by phone, in-home, and/or in the community as needed, based on initial assessment and program practice parameters. Problem-solves by interpreting and adapting guidelines and reviews situational details with supervisor for guidance. Makes decisions based on data interpretation and refines methods and techniques used. Uses judgment to interpret and adapt program guidelines/procedures to client/family needs. Maintains knowledge of program/department/agency goals, target outcomes, and adheres to practice standards and data collection and reporting protocols. Determines processes and methods to follow based on assessment of client/family presentation and strengths; provides client/family-centered support and care. Provides group, individual, and/or family counseling with supervision and coordinates treatment phases with internal and/or external service providers and/or family supports as appropriate. Maintains records of counseling, coordination of care, meetings, and case management activities for reporting and tracking purposes; completes all documentation for client records in accordance with timelines and regulatory requirements. Supports daily program-specific operational functions. May transport clients to appointments or linkages as appropriate. Provides services (evaluation, treatment, wraparound team facilitation, follow-up, case management) in the office and/or community settings; maintains a flexible schedule to meet client needs per program. Provides safety planning and crisis intervention with supervision, including rotating on-call coverage to ensure 24-hour availability as required by the program. Participates in development, implementation, and facilitation of treatment plans and transition plans with defined next steps. Demonstrates knowledge of care levels, referral processes, and resources; makes clinically appropriate referrals in a timely manner. Meets direct service productivity goals as defined by the program and adheres to guidelines and data reporting protocols. Identifies potential risk factors and takes measures to reduce risks in the home using trauma-informed and relationally-based therapeutic interventions. Attends supervision, team meetings, and rounds; seeks additional supervision as needed and follows through with directives. Communicates positively and effectively with clients, families, and internal/external providers. Operates a vehicle in the community as required by program needs. Continues to develop knowledge about the history, traditions, values, family systems, and cultural expressions of groups served, and uses appropriate approaches reflecting cultural understanding. Qualifications
BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required. Previous experience working with a client population with mental health and/or co-occurring substance use disorders. Effective time management and strong verbal and written communication skills are required. Location
