Base pay range $80,000.00/yr - $95,000.00/yr
Role and Responsibilities The RN Case Manager will administer skilled nursing care to patients requiring professional nursing service, teach and supervise the family and other members of the nursing team, and perform these services in accordance with the physician’s orders.
Essential Functions
Provide and coordinate quality patient care, maintain a full case load, and meet productivity goals with consideration of case mix and accurate, timely documentation.
Manage each patient’s care efficiently and cost‑effectively to achieve positive patient outcomes regardless of payer.
Oversee the overall plan of care and outcomes, including care delivered by LPN, HHA, and other nursing staff.
Conduct admission and manage care of patients to the agency by obtaining medical history, performing a physical examination, evaluating the patient, family members, and home situation for health teaching, and determining assistance required for home health aide services.
Interpret nursing and other services of the agency to patients and families as part of planning for care, meet regulatory requirements, practice accepted nursing standards with regard to infection control, and develop and implement the nursing care plan.
Provide skilled nursing care outlined in the nursing care plan, initiate preventive and rehabilitative nursing procedures, observe signs and symptoms, report to the physician, teach, supervise, and counsel the patient and caregivers regarding nursing care needs and other related problems at home.
Oversee care given by home health aides, supervise and evaluate their care, apply documentation standards, and document services rendered to the patient, revise the nursing care plan as needed, and close charts of discharged patients.
Other duties as assigned.
Education Requirements
Associate’s degree in Nursing required.
Qualifications and Requirements
Current, valid RN license issued by NYS Department of Education required.
Minimum one year home care experience.
Knowledge of State and Federal regulations, payer guidelines.
Excellent communication and critical thinking skills.
Flexible and organized.
Ability and desire to serve as a role model.
Working Conditions The RN Case Manager is primarily in a non‑office setting and may be exposed to outdoor conditions. Medium work involves lifting up to 50 pounds occasionally or a negligible amount of force frequently or constantly, and sitting and standing.
