Overview

Advanced Practice Provider (APP) - Colorectal Surgery at Dartmouth Hitchcock Medical Center, Lebanon, NH. The CRS APP will support the Colorectal Surgery division with an emphasis on urgent access, continuity of care, and care coordination across the full disease spectrum—from diagnosis through post-surgical surveillance for complex benign and malignant disease. The CRS APP will collaborate with a multidisciplinary team including colorectal surgeons, gastroenterology, medical and radiation oncologists, nursing, social work, and other allied health providers to reduce clinical backlog, enhance workflows, and implement infrastructure for care coordination and quality tracking. Responsibilities

Provide direct patient care in the outpatient setting at DHMC and Valley Regional Hospitals with a focus on anorectal disease management and long-term cancer surveillance. Conduct new patient consultations, follow-ups, and procedural visits under the supervision of attending colorectal surgeons. Manage and complete patient documentation, orders, and treatment plans in alignment with institutional standards. Serve as a point-of-contact for patients navigating complex care pathways, including surgery, neoadjuvant/adjuvant therapy, and watch-and-wait protocols. Collaborate with the clinical team to balance workloads and reduce provider burnout; mentor and support junior APPs as appropriate. Oversee a new tracking system to monitor oncology patients through staging, initial consult, treatment planning, pre-surgical therapy, surgery, and surveillance, ensuring timely scheduling and monitoring of treatment-related complications. Monitor adherence to clinical protocols, including watch-and-wait paradigms, and flag deviations that may affect outcomes or safety. Coordinate with the clinical research coordinator to maintain the clinical database. Provide triage management and review of urgent referrals for completeness, urgency, and complexity, supporting scheduling as needed. Coordinate with referring providers, radiology, pathology, and scheduling teams to complete necessary diagnostics pre-consult. Act as a clinical contact to improve referral quality and standardize intake protocols, reducing administrative burden on surgical attendings. Graduate of an accredited APRN or PA academic program National specialty certification as FNP, ANP, or NCCPA certification Eligible for NH RN licensure (APRN only) Eligible for APRN or PA licensure in NH Eligible for Federal DEA Registration Minimum of 2-3 years of clinical experience in surgical, oncology, or colorectal care Strong organizational, communication, and critical thinking skills Preferred
