GENERAL SUMMARY: Under the direction of the Clinical Director, Behavioral Health Services, the Social Worker provides psychosocial services to patients and families of all ages with particular emphasis on the emotional and social issues resulting from illness, disability, and/or death.
These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Mercy Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.
Essential Duties & Responsibilities
Documents psychosocial history, initial assessment, appropriate interventions, and expected discharge plan in a comprehensive, clear, timely and legible manner. Reassessment automatically occurs upon notification of significant change in patient’s health status. Coordinates with government agencies. Assists in the development, planning, and facilitation of therapeutic and educational groups for patients/families. Provides clinical social work assessments and services (information, referrals, and counseling) to individuals and/or families within the Mercy Hospital and Medical Center system as well as the general community. Provides services to the patients/families in a professional manner in keeping with the philosophy and mission and principles stated in the Code of Ethics of the National Association of Social Workers. Maintains knowledge of community resources that will enable patients and families to use the health care system and other organizations to their greatest benefit. Provides consultation to the health care team regarding psychosocial management of patient problems or job-related stresses. Additional Duties and Responsibilities Ensures that care is provided for all patients incorporating the attributes of Careful Nursing. Trustworthy collaboration Health Education Perfect skill in fostering safety and comfort Participatory – authoritative management Carries out other duties as assigned.
Qualifications
Master Degree in Social Work from an accredited school required. Licensed as a Social Worker in the State of Illinois required. Knowledge of patients’ growth and development needs. Ability to maintain a high degree of accuracy in carrying out work duties. Ability to cope with stress.

Paid Sick Time - effective 90 days after employment. Paid Vacation Time - effective 90 days after employment. Health, vision & dental benefits - eligible at 30 days, following the 1st of the following month. Short and long-term disability and basic life insurance - after 30 days of employment.
