Nurse Practitioner/Physician Assistant, In-Home Primary Care (Field-based position) - Griffin, GA Village Medical is seeking a Nurse Practitioner or Physician Assistant to join our home-based primary care team serving vulnerable Medicare and Medicare Advantage patients in Griffin, GA, with travel to nearby cities as needed. Must be a graduate and certified from an accredited advanced practice program Two or more years of home care clinical experience as an Advanced Practice Provider or Registered Nurse, with the ability to work autonomously and provide care without physician supervision Comfortable treating adult patients, conducting comprehensive or episodic health histories and physical assessments of patients/families with acute, chronic, or potential health problems Working knowledge of chronic care management and various medication options Current, unrestricted State license or be eligible for licensure and a valid driver's license Active Drug Enforcement Administration (DEA) registration or eligibility Must possess active Medicare/Medicaid enrollment or be eligible to enroll Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) Comfortable with Electronic Medical Records (EMR) system; Athena preferred Familiarity with Microsoft Office About Our Commitment
