VillageMD is seeking a Nurse Practitioner or Physician Assistant to join our home-based primary care team to support our vulnerable Medicare and Medicare Advantage patients in Cumming, GA (travel to nearby cities in the surrounding Cumming area will be required). Provide patient-centered primary care in home-based settings Conduct comprehensive or episodic health history and physical assessments for adults with acute, chronic, or potential health problems Collaborate with a multidisciplinary team to manage chronic conditions and enable care without physician supervision when appropriate Qualifications

Graduate and certified from an accredited advanced practice program Two or more years of home care clinical experience as an Advanced Practice Provider or Registered Nurse, with demonstrated autonomy Comfortable treating adult patients and performing health histories and physicals Working knowledge of chronic care management and medication options Current, unrestricted State license or eligibility for licensure; valid driver’s license Active DEA registration or eligibility Active Medicare/Medicaid enrollment or eligibility to enroll Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification Proficiency with Electronic Medical Records (Athena preferred) and familiarity with Microsoft Office Ability to work independently and remotely as part of a distributed care team Equal Opportunity Employer
