$21.00/hr - $24.00/hr OVERVIEW:

The Patient Benefits Navigator is a key clinic-based role at IMAN’s Federally Qualified Health Center (FQHC). This individual is responsible for assisting patients with insurance enrollment, verifying coverage, and facilitating access to financial assistance programs. The position plays a critical role in expanding access to healthcare services and addressing social determinants of health (SDOH) by providing personalized support and resource navigation. The coordinator will offer comprehensive benefits guidance to ensure patients can effectively navigate available services and coverage options.

KEY RESPONSIBILITIES:

Insurance Enrollment & Benefits Navigation

Assist patients with applications and enrollment for Medicaid, ACA Marketplace plans, and other public or private insurance programs. Verify insurance eligibility using clearinghouse tools and payer portals; update coverage information in the Electronic Health Record (EHR). Provide individualized support to patients navigating community benefit programs such as SNAP, utility assistance, housing support, and unemployment resources. Conduct sliding scale assessments and determine patient eligibility in accordance with organizational policies and federal guidelines. Track application statuses, follow up to ensure approvals, and address denials or gaps in coverage. Stay informed about changes to eligibility criteria, program requirements, and enrollment periods across health and social service programs. Ensure all services are delivered in compliance with federal and state regulations related to confidentiality, privacy, and ethical standards.

Administrative & Collaborative Support

Accurately document all patient interactions, eligibility determinations, and outcomes in the EHR. Schedule patient appointments as needed, ensuring accurate and up-to-date entry of insurance and guarantor information. Maintain and regularly update a resource directory of local, state, and federal programs to support patient needs. Actively participate in team meetings to share insights and improve workflows related to insurance and benefits navigation. Collaborate closely with IMAN’s case management team, PCS Supervisor, and referral coordination staff. Engage in cross-functional teams focused on continuous quality improvement, compliance, and seamless service delivery.

Additional Responsibilities

Support IMAN’s holistic model of care by integrating community wellness initiatives and programming where appropriate. Perform other duties as assigned to support departmental and organizational goals.

REQUIRED QUALIFICATIONS:

High school diploma or GED. Minimum of 2 years of experience in benefits navigation, health insurance enrollment, or case management within a clinical or community-based setting. Strong working knowledge of Medicaid, ACA Marketplace plans, and other public assistance programs is required. Proficiency in using Electronic Health Records (EHR) systems and Microsoft Office Suite (Word, Excel, Outlook). Commitment to improving community health and reducing healthcare disparities. PREFERRED QUALIFICATIONS:

Bilingual in Spanish or Arabic strongly preferred. Certification as a Certified Application Counselor (CAC) preferred

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:

Ability to sit or stand for extended periods of time Willingness to be in the field 3-4 days per week Ability to work for extended hours with a computer Ability to work flexible hours Ability to lift to 50 lbs. Ability to work in extreme temperatures

GENERAL SKILLS AND COMPETENCIES:

Health Insurance Literacy – Deep understanding of Medicaid, ACA Marketplace plans, and other insurance programs, including eligibility and enrollment processes. Benefits Navigation – Ability to guide individuals through complex public benefit systems (e.g., SNAP, utility relief, housing support) with clarity and compassion. Patient Advocacy – Strong commitment to advocating for patients' access to care and social supports, especially those facing systemic barriers. Cultural Competency – Sensitivity to diverse cultural, socioeconomic, and linguistic backgrounds; ability to provide respectful, inclusive support. Communication Skills – Clear and empathetic communication with patients, providers, and partner organizations, both verbally and in writing. Organizational Skills – Ability to manage multiple applications, track documentation, and follow up effectively in a fast-paced environment. Technology Proficiency – Comfortable using Electronic Health Records (EHR), clearinghouse portals, and Microsoft Office Suite. Regulatory Knowledge – Familiarity with HIPAA, FERPA, and other privacy/confidentiality standards relevant to benefits navigation. Collaboration & Teamwork – Works well with clinical staff, case managers, and referral teams to ensure seamless patient care and resource access. Problem-Solving Ability – Resourceful and persistent in overcoming application barriers, resolving coverage gaps, and navigating denials.
