Young Adult Case Manager
role at
Memorial Hospital of South Bend . Cocoon House’s Snohomish Young Adult Housing (SYAH) program provides a minimum of 17 rapid rehousing units to unhoused young adults ages 18-24. Participants may move into an agency-held transitional housing unit while engaged in housing search. The program covers rent initially and gradually shifts to the participant as income and stability increase, with the goal of housing stability at program exit. The SYAH Case Manager works one-on-one with 12-15 young adults to support them through the program, build rapport with participants and local landlords, and assist with the housing and lease-up process while ensuring housing meets program standards. The case manager then supports participants in logistical and practical aspects of renting and maintaining an apartment and helps secure pathways to long-term financial, medical, legal and emotional stability. Establish and maintain a safe, welcoming environment for transitional housing participants.
Support participants in the housing search process, including completing pre-leasing inspections and certifications as required by program policy.
Assist participants in creating and working toward a goal plan, with emphasis on long-term stability.
Collaborate with SYAH team to support participants’ housing stability.
Find and connect participants to needed support services in Snohomish County to facilitate long-term stabilization; provide mobile case management as appropriate.
Facilitate payments to landlords for participant apartments according to the graduated rent subsidy model.
Maintain accurate client records in Cocoon House’s Caseworthy database with timely data entry and uphold confidentiality.
Collaborate with Peer Leader and Program Manager to create client-driven programs.
Attend meetings at program, agency, and community levels as required.
Approach work with a harm reduction and trauma-informed lens.

Dedication to advocating for and working with individuals experiencing homelessness and instability
Lived experience encouraged to apply
Flexibility to adjust schedule to meet client needs, including occasional weekend or evening work
Commitment to serving, supporting, accepting and collaborating with individuals of all races, ages, genders, backgrounds, political opinions and beliefs, sexual orientations, criminal histories, citizenship or marital statuses and religious beliefs
Bilingual applicants encouraged
High school diploma/GED or high school equivalency
Must be 21 years of age or older
At least 1 year of experience working with youth or young adults experiencing homelessness
DCYF background clearance
Acceptable driver’s abstract with no more than 2 violations within the last 3 years
Possess a valid driver’s license, auto insurance, and reliable transportation
Physical
Ability to effectively communicate with all internal and external clients
Be able to hear voices and alarms
Ability to read and comprehend a variety of materials
Ability to operate a phone, computer, and record information for files and logs
Must be able to lift up to 50 lbs
Ability to run up and down indoor and outdoor stairwells in case of youth emergency
