Coordinates an interdisciplinary approach to the physical, emotional, psychological, familial, spiritual, social and nutritional needs of residents. Implements Individual Service Plans and communicates them to the care team; attends service plan meetings and provides nursing updates, including medication review. Manages Activities of Daily Living (ADL) care provided by a team of caregivers, including supervising care, coordinating resident/family requests, handling customer service feedback, and coordinating on-site transportation. Supervises caregivers in Assisted Living, with input from Wellness Nurses and other IDT Members for hiring, training, scheduling, performance assessment, and coaching. Provides supervision to and collaborates with Wellness Nurses to deliver clinical services in an Assisted Living setting. Acts as clinical liaison to healthcare professionals and families as appropriate. Maintains open communication with residents, families, volunteers, staff and other stakeholders. Reviews daily clinical information to provide comprehensive updates to the Director of Nursing and IDT during daily clinical meetings. Assists with documentation including incident reports, admissions, assessments, and evaluations. Audits admissions, transfers, discharges, orders, and EMR data for regulatory compliance and accuracy; corrects information as needed. Ensures compliance with regulatory standards in Assisted Living processes, including weight checks, coordination with dietitian, medication reviews, wound management in partnership with Home Health and Wound Physician, and provider notifications as required. Maintains compliance with regulatory requirements including equipment, narcotic counts, and storage documentation. Oversees Registered Medication Aides with focus on accurate medication administration and ongoing pharmacology education. Maintains a strong focus on safety and risk management at all times. Flexible schedule availability, including periodic Manager-on-Duty shifts, nights, weekends, and on-call as needed. Qualifications
Graduate of an approved RN or LPN program. Current licensure in the Commonwealth of Virginia or compact state of residence. Prior supervisory experience. Minimum of 5 years of clinical nursing experience with a geriatric population. Current CPR certification. Ability to interact tactfully with residents, staff and family members. Excellent reading, writing, speaking and comprehension of English. Proficiency in using computers (Microsoft Office, Microsoft Teams, and web-based platforms for secure management of resident and team member information). Benefits
