As a Home Health Speech Therapist, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community. You will enjoy an autonomous practice focusing on functional rehabilitation within the patient’s actual home environment reflective of your patient’s specific needs. The Home Health environment allows for the time to create relationships with your patients and create your own schedule that is not possible in a hospital setting. Be part of the Banner Home Health Team focusing on getting patients back to their daily life by providing physical and emotional needs during illness and through recovery. This is a Part-Time position with a minimum weekly commitment of 3/8 hour shifts (24 hours per week). Flexible schedule option of 10 hour shifts available. No weekends or Holidays. This position will provide 1:1 home care in the East Valley (Mesa, Apache Junction, Gold Canyon, Superior, Queen Creek, San Tan Valley, Florence, Gilbert and Chandler). The Banner Home Health team is a four-star home care agency with a track record for quality and excellence and is the largest skilled home care agency in Phoenix. Our structure provides support for field clinicians, including a field preceptor, IT specialist, dedicated scheduler, in-office QI specialists, telehealth resources, clinical manager and in-office triage support, to help manage caseloads effectively. Responsibilities

Evaluates patients’ functional abilities and limitations to determine appropriate intervention. Develops a plan of care and treatment goals, as ordered by physician or other referral source. Implements individualized treatment plans that consider patients’ physical, social, spiritual, cultural, educational, and age-specific needs. Ensures plans are supported by standard practices and current literature. Re-evaluates treatment plans as needed for maximum patient benefit. Educates patients, families, and other care providers to promote participation and attainment of therapeutic program goals. Provides individualized home programs with clear instructions. Documents patient treatment according to professional and regulatory standards. Participates in training to other staff and students. Follows plan of care to accomplish functions within established procedures and scope of practice. Exercises independent judgment and consults supervisor for unusual situations. Minimum Qualifications

Master’s degree in Speech Language Pathology from an accredited university program. Current licensure in the state of practice. Certificate of Clinical Competence or documented acceptance in Clinical Fellowship Year. BLS certification required. Ability to work collaboratively with physicians and other healthcare providers. Strong interpersonal and human relations skills. Proficiency with computer software applications commonly used in an automated patient care environment. Arizona Fingerprint Clearance Card at the time of hire and maintained for duration of employment (where applicable). Preferred Qualifications
