Overview

Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a Per Diem RN to join our home care team. This dynamic position involves working directly with an interdisciplinary team in the homecare setting. Must have 1 year of prior experience in acute care, rehab, or homecare. Candidates must be organized, able to teach, and willing to travel between patients. Responsibilities

Provide care for one patient at a time and address the whole patient, including chronic illness self-management. Collaborate with interdisciplinary team members and families to ensure successful patient outcomes. Support transitions from inpatient settings to home health care; participate in ongoing patient education and self-management support. Maintain documentation, coordinate care plans, and promote patient independence after training. Qualifications

1 year RN experience in acute care, rehab, or homecare. Valid New York State RN license. Valid New York State DMV license. Strong communication and critical thinking skills; ability to teach and coordinate with the team and families. Willingness to travel between patients; orientation provided with supervisory support. Employment Details

Employment Type: Part time (Per Diem) Shift: Day Shift Hours: Approximately Monday to Friday, 8:00 am – 4:00 pm; hours and days can be flexible with minimal weekend/holiday rotations. Compensation and Benefits

Pay Range: $36.50 - $49.95 per hour. Pay is based on experience, skills, and education. Orientation and ongoing support from clinical nurse specialists (WOCN, CDE, enhanced palliative care), in-depth computer training, assistance obtaining insurance authorizations, RN supervisors available by phone, and more.
