Job Summary
Overall responsibility of inputting information to the State for the Minimum Data Set (MDS) for each resident admitted to Skilled Nursing of Lions Gate. Job Duties
Inputs initial admission, quarterly, change of statuses and Medicare assessments. Creates care conferences list for all residents on new admissions and quarterly assessments. Submits completed assessments to the State within scheduled timeframes. Oversees all sections of MDS’ for completeness and assures sign-off by the healthcare team. Communicates all information to the Interdisciplinary Team. Facilitates weekly Medicare meetings and is in charge of Medicare schedule with projected dates. Interacts with all health care members – Nursing, PT, OT ST, Dietitian, Activities and Social Services. Coordinates with Social Worker, in advance, monthly care conference schedule. Reviews and updates care plans as needed. Attends morning meetings. Completes managed care updates and coordination. Other duties as assigned by supervisor. Job Requirements
Current license as a Registered Nurse in the State of New Jersey. Knowledge of MDS requirements and Medicare regulations. 3 years’ experience a must. RNAC Certification a must. Competent with computer skills. Ability to communicate effectively in English, both verbally and in writing. Professional Requirements
Adheres to company and departmental personal appearance and dress code policy. Reports to work in a clean, neat and professional appearance. Wears identification at all time while on duty, or representing Lions Gate at functions. Demonstrates dependability and punctuality by reporting to work on time as scheduled. Complies with all Lions Gate policies regarding ethical business practices as outlined in the Corporate Compliance Plan. Maintains and ensures compliance with current requirements, including all State, Federal and regulations including OSHA safety protocol and standards. Completes annual mandatory education requirements (Corporate Compliance, HIPAA, in-service, etc.) Assumes accountability for policies and procedures as contained in the Employee Handbook. Maintains a positive behavior while providing quality services to resident/family members. Follows the preventing resident rights/abuse policies and procedures and follows the established incident reporting procedures. Maintains and ensures resident and facility information as confidential material at all times. Works at maintaining a good rapport and cooperative working relationship with departments, staff, co-workers and management. Represents Lions Gate in a positive and professional manner at all times. Maintains and obeys the dietary laws for maintaining a Kosher environment in identified areas. Attends departmental meetings and in-services as scheduled and required. Accepts assignments in a cooperative and positive manner. Keeps work area neat, clean and orderly. Performs all duties of job to assure resident and personal safety and the protection of co-workers and facility property.
