Summary:
Assists with the implementation of physical therapy services that identifies, prevents, remediates and rehabilitates acute or prolonged physical dysfunction or pain in the patient's home. Under the supervision of a physical therapist, delivers treatment including therapeutic exercise, physical modalities, patient and family education and procedures. Work is performed in the patient's place of residence.
Responsibilities
Works under the direction and supervision of the licensed Physical Therapist in the home environment practice setting.
Performs Physical Therapy procedures and related tasks under the direction of the supervising Physical Therapist.
Provides patient/family education and training.
Performs routine operational functions, including supervision of Home Health Aide.
Documents visits and completes documentation within 24 hours of visit and within agency guidelines.
May modify specific treatment procedures in accordance with patient status within the scope of the established treatment plan.
Participates in Performance Improvement activities and peer review.
Completes continuing education as per agency and state licensing requirements.
Fulfills all compliance responsibilities related to the position.
Performs other duties as assigned.

Other Information
Required: Customer relation skills with patients and staff. All PT modalities, use of a computer.
Minimum Experience: zero to six months
Desired: FIM certification desired.
Working Conditions
Manual: significant manual skills/motor coordination & finger dexterity
