Inpatient Clinical Pharmacist IV, Antimicrobial Stewardship –

Sutter Health . Delivers pharmaceutical care through patient-centered clinical service, medication information, education, medication preparation and distribution, ensuring safe, effective, and cost-efficient medication therapy. Applies sound clinical judgment to contribute to and implement the patient’s plan of care based on the diagnosis. Communicates effectively with patients, families, and healthcare providers while maintaining privacy and safety in accordance with local, state, and federal regulations. Responsibilities

Design, implement, and manage the Antimicrobial Stewardship Program in alignment with local and national guidelines. Advocate for appropriate antimicrobial use and influence prescribing practices to improve patient outcomes and reduce antimicrobial resistance. Track and analyze antimicrobial usage trends; report findings to stakeholders and use data to guide policy and practice improvements. Educate healthcare professionals, patients, and the public on antimicrobial stewardship principles. Provide ongoing training to pharmacy staff and other clinicians. Collaborate with Infection Prevention teams to reduce healthcare-associated infections. Participate in quality improvement initiatives and multidisciplinary rounds; provide direct patient care consultations. Serve as a local representative for the Sutter Health System Antimicrobial Stewardship Program with collaboration on shared goals and projects. Precept PGY1 Pharmacy residents and APPE students. Qualifications

PHARMD — Graduate of an accredited pharmacy school PHARMR — Current registration ACLS — Advanced Cardiac Life Support Typical Experience: 3 years of recent relevant experience Ability to influence physician prescribing at the service-line level Demonstrated skills in managing projects with facility-wide impact Must meet qualifications to precept Pharmacy Residents in an ASHP-accredited Residency Program Knowledge of medical terminology, pharmaceutical calculations, and applicable laws and regulations Knowledge of different Pharmacy practice settings, including narcotic delivery procedures Ability to communicate clearly and professionally with diverse audiences Strong time management and organizational skills; ability to prioritize within policies and procedures Proficiency with computer applications (Microsoft Office, EHR/EPIC) and pharmaceutical technology Ability to work independently and as part of a team; handle multiple tasks with interruptions Maintain privacy of patients’ protected health information (PHI) Job Details
