Overview
Case Manager RN PRN at Portsmouth Regional Hospital - New Hampshire. The Registered Nurse (RN) Case Manager coordinates the plan of care for the patient stay, manages length of stay, ensures appropriate resource management, and develops a safe discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates progression and transition of care using established criteria and works to promote quality outcomes and patient throughput while balancing optimal care with resource utilization. Responsibilities
Provide case management services for both inpatient and observation patients as assigned. Identify patients at risk for adverse outcomes during transitions between levels of care or settings. Perform comprehensive psychosocial, medical, and discharge assessments of patients and families, including assessment of available resources. Reassess patient condition as indicated; develop strategies to mitigate readmission risk through education and community resources. Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary team and patient\'s physician to facilitate a successful care transition. Partner with Social Services to ensure post-acute medical needs and care levels are appropriate; refer timely when risk factors are identified. Evaluate progression of care using evidence-based tools (InterQual) and escalate issues as needed. Make referrals to third-party payers and disease/case management programs for recurring patients and chronic disease states. Facilitate patient throughput with a focus on effective care transition, quality, and efficiency. Document professional recommendations, discharge plans, care coordination interventions, and case management activities for communication across the care team. Align patient needs with available resources to ensure safe discharge/transition. Act as a liaison among physicians, patients/families, hospital staff, and outside agencies; seek ways to control costs without compromising safety and quality. Direct activities to meet the needs of under-resourced patients through education, patient assistance programs, and community resources. Participate in performance improvement activities and adhere to policies, procedures, standards of care, and regulatory requirements. Advocate for patient rights and values, ensuring culturally, ethnically, and religiously respectful care. Perform other duties as assigned. Qualifications
Associate Degree in Nursing or Nursing Diploma Required Bachelor’s Degree in Nursing Preferred 2+ years of experience in case management OR 3+ years of experience in clinical nursing required InterQual experience preferred Registered Nurse license for New Hampshire or compact licensure required Certification in Case Management preferred About Portsmouth Regional Hospital / About HCA Healthcare
