The Registered Nurse (RN) Case Manager (CM) is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team. The CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. Responsibilities include: Provide case management services for both inpatient and observation patients as assigned Identify patients at risk for adverse outcomes during transitions between levels of care Perform comprehensive assessments of psychosocial, medical and discharge needs of patients/family and assess available resources Reassess the patient’s clinical condition as indicated and develop strategies to mitigate readmission risk, including patient education and community resources Coordinate the plan of care and discharge plan with the multidisciplinary health care team and patient’s physician Partner with Social Services to ensure appropriate post-acute medical needs and level of care Ensure timely referrals to Social Services when risk factors are identified Engage patient and family in identifying needs and goals Evaluate progression of care using evidence-based tools (InterQual) and escalate issues as needed Make referrals to third-party payer and disease/case management programs Facilitate patient throughput and safe care transitions Document recommendations, discharge plan, and care coordination activities Align patient needs with resources to ensure safe discharge/transition Act as liaison with physicians, patients/families, hospital staff, and outside agencies Identify opportunities to control costs without compromising safety or quality Direct activities to meet the needs of under-resourced patients and participate in performance improvement Adhere to policy, procedures, and regulatory requirements; follow the Code of Conduct and Mission and Values Advocate for patient rights and respect diverse values Perform other duties as assigned Support ongoing education and growth opportunities within the organization Required Qualifications
Associate Degree in Nursing or Nursing Diploma Required Bachelor’s Degree in Nursing Preferred 2+ years experience in case management OR 3+ years experience in clinical nursing Required InterQual Experience Preferred RN license for New Hampshire or compact licensure required Certification in Case Management preferred Portsmouth Regional Hospital is part of the HCA Healthcare family and serves communities across New Hampshire, Maine, and Massachusetts.
