The Position
As we continue to grow, we are looking for “SNF-ists,” full-time psychiatric nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting and bring quality mental health services to those communities. What You Will Do
As the psychiatric nurse practitioner, you will act as a Skilled Nursing Facility’s leading provider of psychiatric services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents. You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. Responsibilities
Evaluation of new-admission visits Coordination of discharge as needed Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition Other Duties & Responsibilities
Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease Prescribe all necessary medications and treatments Facilitate referrals to other healthcare professionals and medical facilities Communicate with attending physicians and/or specialists regarding patient care Proficiency with documentation and CPT coding Benefits
A healthy work-life balance with flexibility. No need to work overnight shifts or weekends, however as a Vitae provider, you can always take off days during the week and work weekends if you’d like. Malpractice insurance and tale coverage for $1MM/$3MM coverage will be provided by Vitae. Over 4 weeks of paid time off Health benefits and retirement planning with employer contributions. Our Requirements
Advanced Practice Licensure (APN) with an applicable state license PHMNP certification Must have an active license and DEA # Previous experience in nursing or other medical fields Minimum one-year experience in a Skilled Nursing setting (preferred) Ability to build rapport with patients and their loved ones Ability to thrive in a fast-paced environment Excellent written and verbal communication skills Strong leadership experience with emphasis on teamwork in a healthcare setting Education
