Overview
Psychiatric Mental Health Nurse Practitioner, PMHNP role at Telos Health Systems. We are seeking experienced PMHNPs for “SNF-ists” to provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting. Responsibilities
Act as the Skilled Nursing Facility’s leading provider of internal medicine services, collaborating with the interdisciplinary team and physicians, and serving as a day-to-day clinical leader. Oversee the care of all the facility’s residents, working with the facility’s nursing staff and guidance from medical directors and attending physicians. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under protocol for acute and chronic illnesses. Prescribe all necessary medications and treatments. Perform comprehensive physical examinations of patients. Develop and implement patient management policies and procedures. Facilitate referrals to other healthcare professionals and medical facilities. Communicate with attending physicians and/or specialists regarding patient care. Be comfortable and proficient with documentation and CPT coding. Conduct monthly stability visits for long-term residents and evaluations for patients with changes in condition. Support Advanced Care Planning. What You Will Need
Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Active license and DEA number. Minimum one year of experience in a Skilled Nursing setting (preferred). Ability to build rapport with patients and their loved ones. Ability to thrive in a fast-paced environment. Excellent written and verbal communication skills. Strong leadership experience with emphasis on teamwork in a healthcare setting. Coverage area: Wilmington, NC. Schedule: 2 nonconsecutive days a week, Monday–Friday, daytime hours; you pick the days. W2 pay per encounter, fee-for-service PMHNP role. Education: Master’s Degree required. Additional Details
