Job Summary: A Social Services Coordinator is essential in supporting the emotional, social, and psychological well-being of our residents. Services provided meet professional standards of social work practice, consistent with state and federal laws and regulations. Guides facility staff in matters of resident advocacy, protection, and promotion of residents’ rights. In collaboration with the Nursing Home Administrator, allocates resources efficiently and economically to provide medically related social services. The Social Services Coordinator works with hospitals and families, facilitating admissions and discharges at the facility. Markets the facility to the public, hospitals, and other facilities for future admissions. This is a working supervisory position; duties shall be performed as directed and scheduled by the Administrator. Conduct psychosocial assessments upon admission and periodically thereafter Develop individualized care plans in coordination with interdisciplinary teams Provide counseling and emotional support to residents and families Assist residents with transitions, including discharge planning and community reintegration Advocate for residents’ rights and ensure compliance with state and federal regulations Coordinate services such as mental health referrals, guardianship, and financial assistance Facilitate family meetings and care conferences Maintain accurate documentation in accordance with HIPAA and facility policies Support residents in navigating social challenges, including isolation, grief, and adjustment to facility life Collaborate with nursing, therapy, and administrative staff to promote holistic care. Works cooperatively with members of the interdisciplinary team to develop, implement, and evaluate the plan of care. Provides or arranges for social work or other mental health counseling services as needed to attain or maintain the highest practicable mental and psychosocial well-being. Completes admission paperwork with new admissions, family members, or residents. Participates in developing and updating social services policies and procedures that reflect the philosophy and mission of the facility and professional standards of social work practice. Completes all required assessments for MDS quarterly, annually, and upon admission, along with working closely with the MDS coordinator to ensure accuracy. Completes PASSR and level of care through Mountain Pacific upon admission and when otherwise needed per regulations. Collaborate with community organizations, healthcare providers, and other service agencies. All other duties as assigned. Experience in long-term care, gerontology, or healthcare social work preferred Strong knowledge of Medicare/Medicaid, elder care resources, and discharge planning Excellent interpersonal, organizational, and crisis management skills Ability to work independently and as part of a multidisciplinary team Must pass a background check. At least 1-2 years of experience in social services or a similar role. Experience with Point Click Care Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care. Demonstrates a satisfactory level of interpersonal skill to interact with residents, families, community, and government agencies, interdisciplinary staff. Able to express oneself adequately in written and/or oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, staff members, representatives of community and government agencies. Appreciates the importance of maintaining the confidentiality of residents and facility information. Always demonstrates honesty and integrity in the care and use of resident and facility property. Demonstrates ability to prioritize tasks/responsibilities and complete duties/projects within allotted time. Able to respond to change productively and to handle additional tasks/projects as assigned. Able to respond to facility and resident needs on an on-call basis. Able to carry out the essential functions of this job (with or without reasonable accommodation) without posing specific, current risk of substantial harm to health and safety of self and others. Strong communication skills Resourceful problem-solving skills Familiarity with community resources and service providers Experience with data management and reporting software Strong written and verbal communication skills Ability to work independently and as part of a team. Knowledge of the Microsoft platform.
