ACR Health Location: Syracuse Salary: $21.50 Hourly Classification: Non-Exempt, full-time, thirty-five-hour (35.0) week; occasional evenings and weekends required for meetings, workshops, conferences, and seminars. Position Summary

The Housing Case Manager actively assists individuals experiencing housing instability or homelessness by conducting comprehensive needs assessments, developing personalized case plans, and connecting them with appropriate housing resources, support services, and community agencies to secure and maintain stable housing while addressing underlying challenges that may contribute to housing insecurity. Minimum Qualifications

Two-year degree in health/human services or related field; OR three years experience in nonprofit social service agencies providing direct services to diverse populations including persons of color, LGBTQI+ individuals, people who use drugs, individuals with serious mental illness, and those experiencing homelessness or managing chronic illness. Familiarity with housing services and experience working with individuals at risk of or experiencing homelessness is preferred. Demonstrated cultural and linguistic competence, including delivering stigma-free, non-judgmental, and culturally competent services to marginalized groups such as those affected by HIV/AIDS and chronic illness. Strong communication and documentation skills, including knowledge of case note standards for client charts, assessments, and service planning. Proficiency with Microsoft Office Suite, Adobe Acrobat, and web-based systems; experience with Electronic Health Records preferred. Sensitivity to HIV/AIDS, chronic illnesses, and LGBTQI+ issues with a firm commitment to ACR Healths mission, vision, values, and strict adherence to HIPAA compliance. Valid New York State drivers license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership not required). Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F HIV Confidentiality Law, HIPAA Federal Privacy Rules, and 42 CFR Part 2 regulations for protected health information related to substance use. Observe ACR Health policies and procedures, and other relevant regulations for client and agency data security. Protect agency data in accordance with confidentiality requirements. Initiate and complete the process to secure decent, affordable, permanent housing. Provide long-term housing rental assistance and housing retention services to enroll clients in maintaining stable housing to ensure long-term housing security. Complete program enrollments, intake/screening processes, review of program applications, and confirm client eligibility documentation for all enrolled individuals. Conduct assessment and eligibility determination activities, as well as annual home inspections, and income/additional eligibility verifications for all enrolled clients. Provides care coordination and necessary referrals to address all enrolled clients overall health and housing needs. Assist with completing paperwork associated with securing recertification for housing related benefits, income maintenance, and essential housing needs. Assist clients to secure services that maximize their tenure in housing; increase their ability to maintain their household and finances, independently perform activities of daily living, uphold the terms of their lease; prevent lease violations; intervene and mitigate crisis situations; maximize tenant safety and security; and prevent avoidable evictions.
