Non-Exempt, full-time, thirty-five-hour (35.0) week; occasional evenings and weekends required for meetings, workshops, conferences, and seminars. Position Summary:
The Housing Case Manager actively assists individuals experiencing housing instability or homelessness by conducting comprehensive needs assessments, developing personalized case plans, and connecting them with appropriate housing resources, support services, and community agencies to secure and maintain stable housing while addressing underlying challenges that may contribute to housing insecurity. Minimum Qualifications: Two-year degree in health/human services or another related field;
OR Three years’ experience working in a nonprofit social service agency providing direct client-related services to diverse populations (e.g., persons of color, LGBTQI+ community members, people who use drugs, individuals with SMI diagnoses, people experiencing homelessness, chronic illness management, etc.). Familiarity with and experience working in housing or with individuals at risk of or experiencing homelessness is preferred. Access to a reliable and insured vehicle and a valid NYS Driver’s license for travel purposes. Proficiency in Microsoft Office and Outlook; Electronic Health Records systems preferred. Ability to work with diverse colleagues and clients respectfully and with a learning attitude. Effective communication and documentation skills, including familiarity with case note requirements for client chart completion, assessments, and service planning. Experience delivering stigma-free, non-judgmental, and culturally competent services. Sensitivity to HIV/AIDS, chronic illnesses, LGBTQI+ issues, and a strong commitment to the mission, vision, and values of ACR Health. Candidates with lived experience of homelessness, substance use, LGBTQI+ identity, and BIPOC community members are encouraged to apply. Primary Responsibilities: Ensure confidentiality in all duties according to Article 27-F HIV Confidentiality Law, HIPAA, and 42 CFR Part 2 regulations. Follow agency policies and procedures for data security and confidentiality. Assist clients in securing decent, affordable, and permanent housing. Provide long-term housing rental assistance and retention services to maintain housing stability. Complete program enrollments, intake, screening, and verify client eligibility. Conduct assessments, eligibility determinations, annual home inspections, and income verifications. Coordinate care and referrals to address clients' health and housing needs. Assist with paperwork for recertification of housing benefits and income maintenance. Support clients in maintaining housing, managing household and financial responsibilities, and preventing eviction. Advocate for tenants and mediate conflicts with landlords. Provide case management to promote independent living. Update housing assessments and service plans regularly. Coordinate services and referrals through multidisciplinary case conferences. Maintain accurate client records and complete reports timely. Participate in staff meetings, training, and in-services. Maintain knowledge of HOPWA program regulations and policies. Perform additional duties as assigned by supervisors.
