Minimum Qualifications

The incumbent will need to understand as a part of the position travel throughout Area 7 (Orange, Osceola, Seminole) is a requirement and the incumbent must have a valid Florida driver’s license and willing to drive their personal vehicle

1 year of experience fostering relationships, interacting and effectively communicating with community partners

1 year of experience in HIV testing/counseling, HIV case management and/or infectious disease intervention

Possess a current HIV 501 CTL certificate or must successfully complete the HIV 500 and 501 certification class within the first sixty (60) days of employment

1 year of experience in interacting with and effectively communicating with clients/patients from diverse backgrounds

Emergency Duty

Bilingual in either English and Haitian Creole OR English and Spanish

Job Duties and Responsibilities This position is an Early Intervention Specialist (EIS) conducting intensive outreach with the purpose of linking persons with HIV and out-of-care to medical care. Outreach will be focused on initiating, linking, and sustaining optimal medical care for persons with HIV. The EIS will conduct counseling, referral services, linkage to care, health literacy education to enable clients to navigate the HIV system of care.

Engage with community advisory groups to determine service gaps, reasons for these gaps, and reasons for patients dropping out of care. Identify person with HIV who are out-of-care and ensure that they receive assistance during their return-to-care. Work with local County Health Departments and Area 7 HIV Prevention regional staff to identify target populations and target service areas.

Provide Ryan White HIV/AIDS Program (RWHAP) eligibility, HIV counseling, testing, and referral services to assist clients. Ensure linkage to medical care by scheduling doctor’s appointments, ensuring the client attends the initial and follow up appointments.

Provide intensive outreach by connecting with service providers and medical professionals to identify out-of-care persons with HIV and link those persons to medical care. Establish a relationship with clients to ensure successful adherence to their medical care and ensure that clients are retained in care with the assistance and follow up of case management.

Provide health literacy training to clients with HIV to better equip clients with the necessary resources and tools they may need to help them identify and understand the barriers to care that they may face.

Maintain timely documentation of interactions with clients with HIV as well as other work-related activities in the program’s required database/records management system (e.g., HMS).

Maintain relationships with other resource providers, including agencies, organizations, and professionals who provide services for persons with HIV. Accurately code and document delivered services, education encounters, and other pertinent information in clients' electronic medical record. Complete and submit timely all administrative documents. Compile and reconcile data and other information into reports, as requested.

Serve as primary point of contact for linkage to care from hospitals for providing RWHAP eligibility and referral services while clients are inpatient. Serve as a patient navigator from discharge to first appointment with RWHAP medical provider. Collaborate with the hospital’s discharge planner to ensure linkage to outpatient medical care and referrals to psychosocial services.

Skill with computers and software including Windows, Word, Excel, PowerPoint, Outlook, and, if applicable, CAREWARE, STARS, Provide, and other reporting software

Ability to work independently

Ability to maintain client confidentiality

Skill with methods of data collection, analysis and evaluation

Ability to utilize problem-solving techniques

Ability to communicate effectively both verbally and in writing

Ability to establish and maintain effective working relationships with others
