Excel Housing Initiative , we provide supportive housing, individualized case management, and wraparound services that center dignity, stability, and long-term success.
Position Overview RISE is seeking a skilled and passionate
Case Manager
to join our Houston-based team under a
contract-to-hire agreement . This role is ideal for a professional who thrives in a fast-paced, community-centered environment and is experienced in trauma-informed, housing-first service delivery models.
As a Case Manager, you will support youth and young adults experiencing housing instability by developing service plans, navigating housing options, and connecting clients to vital services. After a successful contract period (typically 3–6 months), candidates may be considered for full-time employment.
Key Responsibilities
Conduct in-depth assessments and develop individualized service/housing plans
Provide consistent case management focused on housing retention, employment, and life skills
Maintain accurate documentation in accordance with HUD guidelines and internal standards
Facilitate referrals to behavioral health, education, workforce, and legal supports
Attend community case conferences and housing coordination meetings
Build trusting relationships with clients, landlords, and partner agencies
Document case notes and progress in HMIS or other approved platforms
Assist participants in overcoming systemic barriers to long-term housing stability
Qualifications Required:
Bachelor’s degree in Social Work, Human Services, Psychology, or a related field
2+ years of experience in case management or supportive housing services
Working knowledge of trauma-informed care and youth development principles
Excellent documentation, communication, and time management skills
Proficiency in Microsoft Office and willingness to learn data platforms (e.g., HMIS)
Valid driver’s license and reliable transportation
Preferred:
Master’s degree (MSW or similar)
Experience working with youth transitioning from foster care
Familiarity with HUD programs, coordinated entry, and rapid rehousing models
Experience with motivational interviewing or harm reduction practices
