Overview
Join an established Primary Care practice. The position involves patient assessment, diagnosis based on assessment data and current research, and development and execution of plans of care in accordance with care paths, protocols, standards of care, and current research. Provide education and counseling to patients, families and groups. Clinic hours are Monday - Friday 8am-5pm with excellent work/life balance and no on-call or weekend requirements. Responsibilities
Join an established Primary Care practice Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific and current research Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care and current research Provide education and counseling to patients, families and groups Maintain clinic schedule with Monday - Friday 8am-5pm hours Enjoy an excellent work/life balance with no call or weekend obligations Benefits & Perks

REQUIRED QUALIFICATIONS
Master’s degree or higher in nursing or physician assistant studies from an accredited program Current Illinois license as a nurse practitioner or physician assistant Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA) Minimum of two years of experience as an APP in primary care or a related field is preferred Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications Proficiency in electronic medical record (EMR) systems OSF HealthCare is an Equal Opportunity Employer.
