Day Habilitation Specialist
Full Time, Monday-Friday, Daytime hours
Wage: $18.50 - $19.75/hr
Responsibilities
Actively cooperate with the Program Manager and other DSP staff to achieve the goals and objectives of the Day Habilitation Program.
Implement goal plans, collect data to monitor progress, and prepare monthly progress notes based on the person’s valued outcomes as identified in the ISP.
Schedule and transport individuals to volunteer and recreational activities, providing supervision, training, and support during these activities.
Supervise and evaluate the Day Habilitation Trainer on a periodic basis.
Consult and communicate with all professional staff to ensure individual progress, safety, and regulatory compliance.
Integrate individual program plans into the core room and overall program environment to ensure efficient and habilitative service delivery.
Participate in meetings and regular staffing with the interdisciplinary treatment team for initial, semi-annual, annual, and special reviews, developing program strategies as needed.
Comply with all waiver regulations, Part 624 and 633, reporting violations, deficiencies, and incidents to the Direct Supervisor as discovered or indicated.
Address and report all observed or suspected situations adversely impacting the health and safety of individuals attending the program.
Maintain appearance and cleanliness of the assigned core room.
Maintain and upgrade the core room and program environment to maximize habilitation, progress, and normalization.
Develop individualized goal plans in accordance with the person’s valued outcomes as identified in the ISP.
Maintain accurate daily participant rewards, charts, logs, and case/progress notes per the Direct Supervisor’s instructions and the individual treatment plan methodologies.
Order supplies for the core room with approval from the Direct Supervisor.
Provide lunch and/or transportation duty, feeding individuals.
Assist with participant transportation.
Participate on program or agency committees.
Assist with program projects such as staff training, research, etc.
Cover other core rooms during staff shortages.
Change participants and assist in toileting or showering as appropriate.
Assist during behavioral emergencies, including counseling, verbal calming, supervision, and/or physical intervention.
Requirements
A Bachelor’s Degree in Human Services, Special Education, or a related field and previous experience working with the Developmentally Disabled — OR — a combination of education and experience that meets the needs of the reviewing agencies.
A driver’s license and driving record acceptable to the agency’s insurance carrier.
Candidate must be able to lift 50 pounds and not have any lifting restrictions that would prohibit assistance with toileting, transfers, physical intervention, lifting, etc.
