Day Habilitation Specialist _ OTP Job role at ACHIEVE.

Responsibilities

Cooperate with the Program Manager and DSP staff to achieve program goals.

Implement goal plans, collect data, and prepare monthly progress notes based on the participant’s ISP.

Schedule and transport participants to community activities, providing supervision, training, and support.

Directly supervise and periodically evaluate the Day Habilitation Trainer.

Consult and communicate with all professional staff to ensure individual progress, safety, and regulatory compliance.

Integrate individual program plans into the core room and overall program environment for efficient service delivery.

Participate in interdisciplinary treatment team meetings (Initial, Semi‑Annual, Annual, Special reviews) to develop program strategies.

Comply with all waiver Regulations (Part 624 and 633) and report violations, deficiencies, and incidents to the Direct Supervisor.

Address and report observed or suspected safety‑impacting situations.

Maintain the appearance and cleanliness of the assigned core room.

Upgrade the core room and program environment to maximize habilitation, progress, and normalization.

Develop individualized goal plans in accordance with ISP outcomes.

Maintain accurate daily participant rewards, charts, logs, and case/progress notes per Direct Supervisor instructions.

Program Wide Responsibilities

Order supplies for the core room with Direct Supervisor approval.

Assume lunch and/or transportation duties; feed participants.

Assist with participant transportation.

Participate in program or agency committees.

Assist with program projects (staff training, research, etc.).

Cover other core rooms during staff shortages.

Change participants and assist in toileting or showering as appropriate.

Provide assistance during behavioral emergencies, including counseling, verbal calming, supervision, and physical intervention.

Requirements

Minimum of a Bachelor’s Degree in Human Services, Special Education, or a related field and previous experience working with the Developmentally Disabled (or equivalent education/experience).

A valid driver’s license and clean driving record acceptable to the agency’s insurance carrier.

Ability to lift 50 pounds and no lifting restrictions that prevent assisting with toileting, transfers, or physical intervention.
