$23-24/Hour Position Summary Our Stockton office is seeking a Life Coach III to provide case management and supportive housing services for up to 12 young adults participating in licensed supportive housing programs. Aspiranet’s Transition Age Youth programs offer client-centered, trauma-informed services reflecting best practice standards. Through housing, employment, education, and wellness support, you will help bridge service gaps and foster resilience, self-sufficiency, and long-term success. Aspiranet’s Transitional Aged Youth (TAY) services guide youth 18-25 as they transition from foster care to independent living, ensuring guidance in housing, employment, education, wellness, and other needs for a successful transition. Key Responsibilities Your Skills and Duties: Work collaboratively to engage the Youth and other support team members in identifying strengths and needs to support the development and implementation of a service plan. Provide weekly case contact and support to assess program compliance, regulatory standards, and progress. Conduct assessment and screenings to ensure identified needs and safety issues are addressed via appropriate referrals, follow-up, and safety planning. Qualifications Education & Work Experience Bachelors of Arts or Science degree in a related field, 2 years of experience working with children and/or transition age youth and knowledge of the foster care system. Licenses & Certifications Meets all state required conditions of employment as set forth by funder and regulatory body; e.g., Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid CDL and clean driving record.
