Vanguard Skin Specialists is a growing, physician-led medical group with a bold and purpose-driven mission: to deliver outstanding care while building a scalable, sustainable platform for multi-specialty services. With 9 locations, ~150 employees, and expanding operations – including a dermatology/plastic surgery/aesthetic services medical practice, an ambulatory surgery center in development, a real estate portfolio, and a retail skincare business – we are entering our next phase of growth. We are seeking a results-driven Controller who will take full ownership of our finance and accounting function, helping us build the financial infrastructure to support sustainable growth. The Controller will lead financial operations across multiple business lines and report to the CEO as a core member of the executive team. The Opportunity
As the Controller, you will lead financial operations across multiple business lines, including our clinical practice, real estate holdings, retail skincare business, and upcoming ambulatory surgery center (ASC). This role combines building and leading—able to dive into accounting details one day and present strategy to executive leadership the next. You will work closely with the COO and fractional CFO on strategic financial initiatives. You will report directly to the CEO and serve as a core member of the executive team, helping shape our financial roadmap while building the infrastructure to scale. Responsibilities
Lead budgeting, forecasting, and financial modeling across all business entities. Build and maintain dashboards, KPIs, and executive reports to drive decision-making and monitor the financial health of the organization and each business. Partner with CEO and COO on margin improvement, capital planning, and capital allocation strategy. Lead cost structure analysis to identify margin improvements through cost controls, revenue strategy, or operational efficiency. Partner with CEO and COO for scenario models and ROI analyses for new investments, partnerships, and service lines. Implement rolling 12-month cash flow forecast. Accounting & Reporting
Develop a high-integrity, efficient accounting foundation to support multi-entity operations and future audit-readiness. Own the month-end close, general ledger oversight, and internal controls. Generate accurate and timely financial reports including Balance Sheet, P&L, and Statement of Cash Flows across all entities. Collaborate with managers to develop budgets and forecasts for business units and manage the practice to meet or exceed budget goals. Report significant variations from prior periods or budget with commentary and corrective actions. Ensure compliance, tax readiness, and audit preparedness. Oversee policies for expense tracking, revenue recognition, and entity-level allocations. Lead reporting of key operational metrics including provider dashboards and organizational KPIs. Internal Controls & Processes
Ensure scalable systems and processes that protect cash flow and reduce administrative overhead as volume grows. Establish and monitor internal control policies and procedures. Maintain GL with supporting subsidiary ledgers and regular reconciliations. Establish appropriate segregation of duties and compensating controls where needed. Oversee A/R and A/P processes to optimize cash flow. Oversee payroll, taxes, benefits, and related reporting; manage 401K, profit share, and PTO. Oversee banking relationships and treasury policies. Real Estate & Capital Projects
Oversee financial operations and reporting for real estate portfolio. Lead ASC development project financials, including pro forma modeling and budget oversight; coordinate with legal, clinical, and construction teams. Oversee cost segregation for real estate properties. Manage property tax assessments and appeals to reduce tax burden. Ensure all real estate assets are insured with regular coverage reviews. Provider Compensation
Calculate, analyze, and administer provider compensation and year-end bonuses aligned to policy and performance goals. Maintain dashboards and reporting tools for provider productivity, expense allocations, and compensation metrics. Engage providers and leadership in data-driven discussions on collections and compensation. Team Building & Growth
Build systems, processes, and reporting to support a growing MSO and diversified portfolio. Participate in executive-level strategic conversations; evaluate financial implications of strategies. Design and manage a lean, high-impact finance team; plan hiring as needed. Model Vanguard’s values and support the mission to impact patients and community positively. Location: Onsite at headquarters in Colorado Springs, CO. Occasional travel to other locations may be required. Not remote or hybrid. Executive presence and responsiveness are expected.
