Job Summary CNCM I facilitates transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I is accountable for patient's caseload and plans effectively in order to meet patient needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote efficient utilization of resources. CNCM I participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.
Minimum Qualifications
Degrees:
Graduation from an accredited school of Nursing with a Bachelor's degree required.
License & Certifications:
Register Nurse; Licensed to practice nursing in the State of Texas.
Related specialty certification preferred
Basic Life Support
Work Experience:
Three (3) years experience as RN in appropriate area assigned required;
One (1) year experience in Case Management, Quality Management, and Coding preferred.
Management Experience:
One (1) year leadership/clinical experience preferred.
Communication Skills:
Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking), Writing/ Correspondence, Writing/ Reports
Proficiencies: MS Word, P.C.
