Support the healthcare team in transitional care by acting as a liaison between patients, their families, doctors, and health care providers. Facilitate transitional care to enhance the quality of clinical outcomes and patient satisfaction. Identify areas for performance improvement (e.g., day-to-day workflow, process improvements, patient satisfaction) and drive changes. Manage patient caseload and plan effectively to meet patient needs across the continuum of care; provide family support, manage length of stay, and promote efficient utilization of resources. Advocate for patients and coordinate care with other health care professionals to determine goals and develop both short-term and long-term plans. Participate in quality improvement activities; exemplify professionalism; promote a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff. Qualifications
Education: Graduation from an accredited school of Nursing with a Bachelor\'s degree required. Licensure: Registered Nurse; Licensed to practice nursing in the State of Texas. Related specialty certification preferred. Certifications: Basic Life Support. Experience: Three (3) years experience as RN in appropriate area assigned required; one (1) year experience in Case Management, Quality Management, and Coding preferred. Management Experience: One (1) year leadership/clinical experience preferred. Communication Skills: Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking), Writing/Correspondence, Writing/Reports. Proficiencies: MS Word, PC. Job Attributes - Knowledge/Skills/Abilities: Analytical, Medical Terms, Mathematics. Work Schedule: Weekends, Holidays, Flexible, Travel, On Call. Equipment Operated: PC and typical medical equipment related to job duties.
