Job Summary
This is an essential position which directly impacts the quality of life of each resident. Responsibilities include but are not limited to delivering personal care to residents, serving meals and snacks, assisting with activities and housekeeping as assigned. Care Manager focuses on maintaining an environment for socialization and independence. All actions must display respect for residents, family members and staff. Responsibilities
Ensure that the Victory Housing Mission Statement takes precedence over all decisions and actions. Adhere to all Victory Housing policies and procedures, COMAR 10.07.14 regulations, and County, State and Federal guidelines (including DOH, CDC, CMS and OSHA). Implement universal precautions and infectious disease control protocols at all times and wear assigned PPE as policy and requested. Complete all assigned training including initial orientation, COMAR 10.07.14 requirements, monthly in-services, HIPAA, VHI required personal care and skills modules; participate in required safety and disaster drills as assigned and mandated. Provide personal care needs, emotional support, and social stimulation to residents. Utilize Eldermark platform to adhere to assigned resident service delivery and daily tasks; document completion of tasks using the assigned IPAD. Assist residents according to ISP and CareTracker; deliver care unique to each resident based on preferences, abilities, and expectations. Services include, but are not limited to: Shower/Bath as assigned Dress or assist with dressing (or undressing) using clean clothing Brush hair; apply makeup Brush teeth; clean dentures Assist with shaving Assist with toileting as needed; remind residents as appropriate Assist residents with making beds; straighten rooms; perform housekeeping tasks in the resident a0suite as needed; keep rooms tidy and floors clear Toileting completion documented in Eldermark; note unusual behavior in Progress Notes and notify RN/LPN/Coordinator Serve meals in the dining room with hospitality and customer service; deliver meals on trays to residents in their suites if required In the dining room, assist with seating; set tables per posted diagram; ensure condiments and beverages are available; prepare beverage stands; position tray stands; verify all residents are seated before meal service Assist in serving meals according to serving pattern or assigned tables; greet residents and note their meal preferences and portion sizes Serve meals at safe, allowable temperatures; cover and store according to safe food handling practices Provide seconds or assist with individual needs (e.g., cutting meat); offer beverage refills throughout meals Clear dishes after each course; offer diabetic desserts if appropriate Assist with post-meal cleanup: vacuum/sweep/mop dining room as needed; clean chairs; restock Victory Drink, Snack and Laugh Station; check station every two hours; clean/sanitize as needed; transport dirty items to kitchen; remove trash as needed Prepare coffee and tea; ensure fresh snacks are available; maintain clean glasses and plates Announce activities; create a fun environment and assist residents needing help to participate Lead and participate in daily activities; encourage participation Respond to help and call bells within 3 minutes; take appropriate action Answer incoming calls professionally and document messages with essential details Placesoiled clothing in laundry bag; wash clothes as needed Provide physical support as needed (e.g., ambulation, assistance from floor after a fall, transfer to/from bed or bath/shower, assistance to evacuate during an emergency); wheelchairs used on a limited basis as directed by PT/OT; promote independence and dignity Document legibly in English in the Communication Log; read and certify by signing and dating before each shift commences Provide individualized one-on-one activities and services for Residents as assigned and as time allows (e.g., reading, writing letters) Encourage self-care and independence in coordination with Health, Wellness, RN/LPN/Coordinator Promote social interaction with Residents, family members, or staff Perform special duties such as cleaning laundry rooms or storage closets Maintain a safe environment by following safety procedures for lifting and transferring, fire, disaster response, infection control, chemical storage, and reporting unsafe conditions Report equipment malfunctions and assist housekeeping as needed Maintain positive public relations and a collaborative, supportive team atmosphere Perform other tasks as assigned by supervisor Attend monthly staff meetings and scheduled training sessions Job Qualifications
One year of experience in caring for geriatric residents; certified nurse aide experience preferred Team player with good communication skills, caring attitude, and genuine concern for seniors Must speak, read, and write in English Must be current on all vaccinations including COVID-19 and receive CDC-recommended booster doses
