Job Overview
The
Change Management Director
will lead day-to-day strategy and execution of organizational change management initiatives at
Premise Health . This hybrid, full‑time role is located in Brentwood, TN and partners with senior leadership and cross‑functional teams to drive people, process and technology transformations that align with strategic goals.
What You'll Do
Assist in the development and execution of enterprise change management strategies for large‑scale organizational, cultural and technology transformations.
Partner with senior leadership to identify change impacts, assess organizational readiness and build tailored strategies to drive adoption.
Establish and maintain enterprise‑wide change management frameworks, methodologies, tools and best practices.
Lead planning and execution of critical initiatives, including stakeholder engagement, communications, training and reinforcement.
Collaborate with HR, Communications and functional leaders to design programs that support employee adoption and minimize resistance.
Coach executives, managers and project leaders to strengthen change leadership capabilities.
Measure effectiveness of change initiatives using KPIs, adoption metrics and employee feedback, and adjust strategies for continuous improvement.
Build and lead a high‑performing change management team, fostering professional growth and knowledge sharing.
Serve as a trusted advisor to senior leaders, providing insight and recommendations to align change initiatives with business priorities.
Champion a culture of agility, innovation and resilience across the organization.
Assess organizational readiness at key milestones, define and track adoption KPIs, and report progress.
What You'll Bring
Bachelor’s degree (preferred) or equivalent work experience.
Change Management certification (Prosci).
10+ years of progressive experience in change management, organizational development or transformation leadership.
Experience building and leading high‑performing teams.
Proven track record of leading enterprise‑scale change initiatives with measurable success.
Deep knowledge of change management methodologies (e.g., Prosci, Kotter, ADKAR) and organizational design principles.
Strong leadership and influence skills with the ability to engage and inspire stakeholders at all levels.
Exceptional communication, facilitation and presentation skills.
