Speech-Language Pathologist (SLP) - Home Health - Part Time - Lincoln/Lancaster County
Tabitha's Home Health in Lincoln, NE is seeking a Speech-Language Pathologist to work 24 hours per week, Monday to Friday. The Speech-Language Pathologist is responsible for identifying, assessing, and treating clients with speech, language, swallowing disorders, and voice disorders.
Essential Functions
Evaluates the client by utilizing appropriate diagnostic assessment tools for dysphagia, speech/language/voice disorders.
Performs Speech Pathology treatment procedures based on initial and ongoing client evaluation.
Develops and implements the client's plan of care in collaboration with the physician, other health care professionals, family/caregiver according to agency procedures and state and federal regulations.
Documents the care provided, instructions given, client's progress toward goals, and changes in condition following each visit adhering to Tabitha's policies and procedures and state/federal regulations.
Reports significant changes in the client's condition to the physician and nursing staff.
Communicates and coordinates care with other health care professionals involved in the care.
Designs, implements, and evaluates therapeutic interventions and instructs the client’s family/caregiver/Rehabilitation Aide/Functional Maintenance Aide in carrying out program.
Consults with the attending physician and obtains a written telephone order for changes in the client's plan of care.
Ensures that assigned clients' clinical records are complete, timely, and reflect coordination of care.
Oversees and coordinates Rehabilitation aides client care activities.
Organizes and prioritizes work to complete responsibilities according to plan of care within established time frames to meet the client’s functional, medical and social needs.
Ideal Candidate
Graduate of an accredited Speech Language Pathologist school.
One year experience as SLP; geriatric experience preferred.
Speech Language Pathology license in good standing with Nebraska State Registry; Certificate of Clinical Competence from the American Speech and Hearing Association. CPR required. Valid Nebraska Driver’s license and good driving record. Auto insurance at or above statutory requirements. Must follow Tabitha’s vaccination requirements.
Must speak, read, write, and understand English including medical terminology. Ability to follow instructions, provide instruction, read and understand care plans, interact with seniors/clients/caregivers, hold conversations, compose professional emails, etc.
Physical requirements: ability to perform duties while moving around, lift, push, pull, reach, squat, kneel, bend, carry supplies, frequent periods of intense activity, maintain calmness during emotional situations.
Age at least 18.
