Speech‑Language Pathologist (SLP) – Home Health – Full Time – Lincoln/Lancaster County Join Eventide Senior Living Communities as a Speech‑Language Pathologist working 8:00 am–5:00 pm, Monday to Friday. The SLP identifies, assesses, and treats clients with speech, language, swallowing, and voice disorders.

Responsibilities

Evaluate clients with appropriate diagnostic assessment tools for dysphagia, speech/language/voice disorders.

Perform Speech Pathology treatment procedures based on initial and ongoing client evaluations.

Develop and implement the client’s plan of care in collaboration with physicians, other health‑care professionals, and family/caregivers according to agency procedures and state/federal regulations.

Document care provided, instructions given, client progress toward goals, and changes in condition following each visit, adhering to Tabitha’s policies and procedures and regulations.

Report significant changes in client condition to the physician and nursing staff.

Communicate and coordinate care with other health‑care professionals involved in the care.

Design, implement, and evaluate therapeutic interventions and instruct the client’s family/caregiver, Rehabilitation Aide, or Functional Maintenance Aide in carrying out the program.

Consult with the attending physician and obtain a written telephone order for changes to the client’s plan of care.

Ensure that assigned clients’ clinical records are complete, timely, and reflect coordination of care.

Oversee and coordinate Rehabilitation aides’ client care activities.

Organize and prioritize work to complete responsibilities within established time frames.

Ideal Candidate

Education:

Graduate of an accredited Speech Language Pathology program.

Experience:

One year as an SLP (preferred); geriatric experience preferred.

Credentials:

Speech Language Pathology license in good standing on the Nebraska State Registry; Certificate of Clinical Competence from the American Speech and Hearing Association; CPR required; Valid Nebraska driver’s license; auto insurance at or above Nebraska statutory requirements.

Communication:

Proficient in spoken and written English, including medical terminology.

Physical Requirements:

Ability to drive to client homes and perform duties requiring lifting, pushing, pulling, and continuous activity throughout the shift. Must pass a 25‑pound physical exam.

Age Requirement:

At least 18 years old.
